Everything looks correct except:

0 = Peripheral
1 = Telecom
2 = Backplane

I cannot see 15 good enough (I will need to see the other side of the board).

30 = Midgrade

Correction, 30 is Low Grade and 15 is in fact Telecom. (I accidently skipped over the front view picture the first time through).

The reason 30 is lowgrade is because of the brown side and lack of any gold content be it pins or plating.
